Dopamine agonist therapy for hyperprolactinaemia, such as with bromocriptine, leads to a reversal of the hyperprolactinaemic hypogonadal state without risk of the development of pituitary insufficiency. Dopamine agonist therapy is effective not only in patients with microadenomas, but also in the majority of patients with large prolactin-secreting tumours in reducing tumour size; however, the tumour size will increase (as will prolactin levels) after withdrawal of therapy. This may give rise to problems due to compression of vital structures by the tumour.
